Starting with best condition (same double A for CBs, perma +1)

At half-time:
sub out DMF for the best condition.

Timeout at 55'~60' (or when ball out/foul):
Swap LB with left CB
Swap RB with right CB
and use best condition as the sub for CBs (same double A preferred but if those are blue/purple, you can pick anyone in the bench)

If extra time:
sub out DMF for the best condition
